You are required to verify your booking appointment request via text message or email. We will contact you to confirm the booking during our staffed hours to process a $50 booking deposit that will go towards the cost of your treatment.
Please ensure you contact us two business days in advance if you are unable to attend your appointment to avoid a late cancellation fee or your deposit being forfeited.
New patients should arrive early to their appointment to complete a comprehensive medical history form. Alternatively, you can complete the form online, before your appointment. We advise bringing a list of your current medications/allergies to your appointment to ensure we carry out dental treatment safely.
Please ensure you bring your private health insurance card to utilise our onsite on-the-spot HICAPS claiming facilities. The health fund rebate you receive is determined by your level of coverage and agreement with your health fund. We will provide a treatment plan estimate on the day of treatment and check your health fund contribution if you are eligible.
Some treatments may require longer or additional appointments. These procedures include but are not limited to: the treatment of extensive decay and gum disease, emergency treatment, Invisalign, oral surgery, dental implants, root canal treatment and cosmetic dentistry. The team at Simply Teeth will advise you if this is required.
