All you need to do is fill out the form or use the referral card to introduce your family, friends or colleagues to us. Once they have completed their first visit, your account will be credited.

This credit can be used towards any treatment you choose. Please note that this credit cannot be exchanged for cash and must be used in the next 12 months of the credit date.
